Our new patient forms can be downloaded and completed in the convenience of your own home.
Our goal is to make you feel comfortable and to provide a great first impression. Your first visit in our office is just that….YOURS! It’s YOUR opportunity to tell us why you’ve come in, what is important to YOU and what YOUR health goals are. This starts when our staff presents your new patient paperwork which includes questions about you and your health. Answers to these questions help us determine where the rest of your first visit goes.
Our Chiropractic Evaluation
Typically, our chiropractic evaluation procedures involve a detailed consultation and history of your health challenges or concerns, a brief summary of your lifestyle stresses and choices, a thorough physical examination, nervous system scanning to assess underlying levels of stress and x-rays to check on the wear and tear on your spine.
Follow up Appointment
Once completed, we will then schedule a follow up appointment where you and your Doctor will review the results of your tests. Options for care will be customized to your needs and presented to you during this appointment. Typically your care will begin during this appointment, but only after you feel satisfied with the explanation of what is happening and your questions have been answered.
